Scheduling an Exam

CML HealthCare offers short wait times and rapid reporting of results.

To schedule an examination we require a patient requisition form, signed and completed by your physician. This form is available online and may be sent to any of the clinics below. Upon receipt of the patient requisition form, a representative from CML HealthCare will contact you to schedule your appointment.

If you need to reschedule your exam, please be sure to call our office as soon as possible so that we can book your appointment for a later date. If you do not call to cancel 24 hours prior to your exam, a cancellation fee may apply.
